- Some people have eyelid surgery to correct problems that are a result of aging, while others have inherited traits such as under eye bags that cause them to seek treatment in their 20s or 30s.
- If you have excessive or unsightly lower eyelid skin.
- If you have puffy pouches of fat in the lower lids that make you look tired.
- If the lower lid margin sags downward at the outer corner.

- Restores a firmer, smoother, and more rested look to the lower eyelid and improves the shape of the lower eyelid if it is sagging downward.

The surgery is done on an outpatient basis under light general anesthesia or sedation with local anesthesia.
Dr. Mosher will discuss two different approaches to lower lid surgery: one uses an external incision placed near the lash line and the other
(transconjunctival blepharoplasty) is performed from inside the lower lid so there is no scar on the outside. The eyelid skin tone, extent of the fatty deposits, and your goals will determine which approach is used.
- There is usually only mild discomfort following this procedure.
- Bruising and light
sensitivity will last from five to ten days. Swelling
rapidly diminishes in one to three weeks.
- Makeup and contact
lenses can usually be worn in four to six days.
- You should avoid exercise, lifting, bending over, and straining for
two weeks.

Laser resurfacing
of the lower eyelid and crow's feet is often done in conjunction with transconjunctival
blepharoplasty.

Upper eyelid,
brow, and
facelift surgery can often enhance the potential for improvement.
- Many patients request lower eyelid surgery to get rid of dark circles under the eyes.
Dr. Mosher
can discuss with you the best approach to improving the appearance of these. Dark circles are often caused by a combination of factors.
- Bulging fat pockets will create a shadow effect that contributes to the dark circles. This is apparent when you look at how different types of lighting affect these circles. Office overhead lighting often accentuates this problem whereas direct sunlight makes the circles less obvious.
- Many patients inherit a "hollowness" or "trough" in this area that again accentuates the problem. In this instance, filling in the trough with an implant or with fat transplantation may be required.
- Finally, some patients have lower lid skin that is very thin and almost transparent. This makes the skin look gray or blue and accentuates the dark circles.
Skin treatments,
microdermabrasion, and
laser resurfacing
can be used to improve the appearance of this skin. This is why wearing concealer eye cream helps as well. The concealer causes more light to reflect off of the skin making the skin less transparent and more uniform in color.
- Usually a combination of lower eyelid surgery and
skin treatments offers the best solution to this problem.
